Chemical Health Assessment
To enter Avivo’s treatment programs, you must have a chemical health diagnosis, which is documented by a Chemical Health Assessment. This assessment provides access and funding for substance abuse treatment.
In-person assessments have resumed on a first-come first-served, walk-in basis, Monday – Friday, at our Minneapolis location at 1900 Chicago Avenue. Doors will open at 6:30 a.m, November – March, and 7:00 a.m., April – October.

Addiction Test
Are you wondering if your alcohol or other drug use is becoming a problem?
This four-question addiction quiz* helps to reveal the possibility of substance abuse.
- Have you ever felt you should cut down on your drinking or drug use?
- Have people annoyed you by criticizing your drinking or drug use?
- Have you ever felt remorseful about your drinking or drug use?
- Have you ever had a drink or used drugs in the morning in order to relieve a hangover or steady your nerves?
Answering “yes” to two or more questions may indicate a problem with alcohol or other drugs.

Clinicians and counselors at Avivo will not assume that individuals have a diagnosis of substance abuse when they reach out. Our Chemical Health Assessment provides an accurate diagnosis along with recommendations of the care that’s right for you.
The chemical health assessment is the professional assessment individuals need to consider residential or intensive outpatient treatment if that is the next step. Our professional staff is here to help you.

Community Support Programs
Membership to Avivo’s Community Support Program is free and open to adults residing in Hennepin County who live with a severe and persistent mental illness. We offer a place to belong, build friendships, connect with others – while experiencing engaging activities and finding meaningful life resources! Avivo CSP programming is offered in-person and virtually.
Structured and welcoming wellness activities are offered in-person at two gathering place locations in Minneapolis. Staff offer ArtWorks, Avivo’s widely known, inclusive arts programming founded on the principles of arts access, recovery, and social justice. Avivo’s gathering places also offer wellness activities that include nutrition, physical activity, games, fellowship, staff support to connect to community resources, and more.

Hennepin County
Where to find housing
Avivo receives referrals for it’s housing programs through a process called Coordinated Entry.
Staying in homeless shelters
If you are currently staying at a homeless shelter in Hennepin County, you will be identified by shelter staff for assessment.
Please note that anyone who accesses emergency shelter for 14 days is eligible for an assessment and should request one from their shelter worker or street outreach worker if not already scheduled.
Fleeing or attempting to flee domestic violence
If you are an individual who meets all of the following criteria:
- Is fleeing, or attempting to flee domestic violence,
- Has no other residence
- Lacks the resources or support networks to obtain other permanent housing

Staying outdoors, in a vehicle, camping, etc.
In Hennepin County, single adults should contact the Hennepin Shelter Hotline at 612.204.8200 Monday-Friday, 8 a.m. – 10 p.m., or reach out to a Street Outreach worker they may be working with. After hours, please call United Way 2-1-1 (mobile: 651.291.0211).
How is someone referred to Avivo Village?
There is not a “direct” referral process to Avivo Village. It is designed for individuals living unsheltered who are not using the traditional shelter system and are living outside or other places not suitable for living. Many outreach teams and other providers working in Hennepin County utilize HMIS (Homeless Management Information System) to designate an individual, fitting Avivo’s target population, as preferring Avivo Village.
Ramsey County
Ramsey County family and individual access can be found here. If you and your family don’t live in Hennepin County or Ramsey County, there are still options for you. All counties in Minnesota have some form of Coordinated Entry.
